What Is Liver Failure and Why Does It Happen?
The liver is the body's largest internal organ, responsible for filtering toxins from the blood, producing bile for digestion, synthesising proteins, and regulating metabolism. When the liver is severely damaged and can no longer perform these functions, it is called liver failure — a life-threatening condition that may require a transplant.
Liver failure can develop slowly over years (chronic liver failure) or suddenly within days (acute liver failure). Chronic liver failure is far more common and is usually the end stage of a long-standing liver disease.
- Cirrhosis — scarring of the liver caused by long-term alcohol abuse, hepatitis B or C infection, or fatty liver disease (NAFLD/NASH)
- Viral hepatitis B and C — chronic infection that progressively destroys liver cells over 10–30 years
- Autoimmune hepatitis — the immune system attacks liver cells
- Wilson's disease and haemochromatosis — genetic disorders causing copper or iron buildup in the liver
- Primary biliary cholangitis (PBC) and primary sclerosing cholangitis (PSC) — bile duct diseases
- Biliary atresia — a congenital condition in children where bile ducts are absent or blocked
- Acute liver failure from paracetamol overdose, drug toxicity, or sudden viral infection
Warning Signs: When Your Liver Is Struggling
Liver disease is often called a "silent" disease because symptoms appear only when significant damage has already occurred. Knowing the warning signs can save your life.
- Jaundice — yellowing of the skin and whites of the eyes (caused by bilirubin buildup)
- Ascites — swelling of the abdomen due to fluid accumulation
- Oedema — swelling of the legs and ankles
- Extreme fatigue and weakness that does not improve with rest
- Dark urine (tea-coloured) and pale or clay-coloured stools
- Easy bruising and prolonged bleeding (liver no longer makes clotting proteins)
- Confusion, memory problems, or personality changes (hepatic encephalopathy)
- Vomiting blood or passing black tarry stools (bleeding from oesophageal varices)
- Persistent nausea, loss of appetite, and unintentional weight loss
How a Liver Transplant Is Performed
A liver transplant replaces a diseased liver with a healthy one from a donor. India offers two types: living donor liver transplant (LDLT) and deceased donor liver transplant (DDLT). Living donor transplant is more common in India because deceased donor organs are scarce.
In a living donor transplant, a healthy family member or compatible donor donates 60–70% of their liver (the right lobe). Both the donor's remaining liver and the recipient's new liver regenerate to full size within 6–8 weeks — the liver is the only organ in the body that can fully regenerate.
The surgery takes 8–12 hours for the recipient and 4–6 hours for the donor. The recipient is placed on immunosuppressant medications for life to prevent rejection.
- Pre-transplant evaluation: blood tests, imaging (CT/MRI), cardiac assessment, and psychological evaluation for both donor and recipient
- Donor surgery: right lobe of the liver is removed laparoscopically or through open surgery
- Recipient surgery: diseased liver is removed and the donor lobe is connected to the recipient's blood vessels and bile ducts
- ICU stay: 5–7 days in intensive care for close monitoring
- Hospital stay: 3–4 weeks total before discharge
- Recovery at home: 4–6 weeks before flying back to home country
- Lifelong immunosuppressants: tacrolimus, mycophenolate, and steroids to prevent rejection
Life After a Liver Transplant
Most liver transplant recipients return to a normal, active life within 3–6 months of surgery. The 1-year survival rate at top Indian centres is over 90%, and the 5-year survival rate is 75–80% — comparable to the best centres in the USA and Europe.
Patients must take immunosuppressant medications every day for life. These medications prevent the immune system from rejecting the new liver but also increase the risk of infections and certain cancers. Regular follow-up blood tests every 3–6 months are essential.
- Return to work: most patients return to desk jobs within 3–4 months
- Diet: healthy, low-sodium diet; avoid alcohol completely for life
- Exercise: light walking from week 2; normal exercise by month 3–4
- Pregnancy: possible 1–2 years after transplant with specialist guidance
- Avoid raw/undercooked food and crowded places for the first 6 months (infection risk)
- Annual cancer screening (skin, lymphoma) due to immunosuppressant use

Frequently Asked Questions
How much does a liver transplant cost in India?
A living donor liver transplant in India costs $25,000–$35,000 USD total (including recipient surgery, donor surgery, hospital stay, and initial post-op care). This is 75–80% less than the USA where liver transplant costs $200,000–$500,000.
Which hospital is best for liver transplant in India?
Top hospitals for liver transplant in India include Medanta The Medicity (Gurugram) — one of Asia's largest liver transplant centres, Apollo Hospital (New Delhi), and Max Super Speciality Hospital (Saket). All have dedicated hepatology and liver transplant teams.
Can I bring my own donor for liver transplant in India?
Yes. India allows living donor liver transplants from close relatives — spouse, parents, siblings, children, and in some cases first cousins. The donor donates 60–70% of their liver, which regenerates fully within 6–8 weeks. We assist with all legal paperwork and Authorization Committee approval.
How long does a liver transplant patient need to stay in India?
Liver transplant patients typically stay 4–6 weeks in India. The recipient stays 3–4 weeks in hospital and 1–2 weeks for recovery before flying. The donor stays 7–10 days. We recommend staying at least 5–6 weeks total for proper post-transplant monitoring.
Is paediatric liver transplant available in India?
Yes. Paediatric liver transplant for children with biliary atresia, Wilson's disease, and other metabolic liver diseases is available at Medanta and Apollo. Paediatric liver transplant costs $22,000–$30,000 USD — significantly less than USA or Europe.
What documents are needed for liver transplant evaluation?
Send us recent liver function tests (LFT), bilirubin, INR, albumin, MELD score if available, ultrasound or CT scan of liver, and any previous hepatologist reports. We will get a transplant evaluation from the specialist within 24–48 hours.
